Case Manager, Contract - Part-Time ONLY: REMOTE
Opportunity for a qualified Case Manager who can provide a high level of individual patient management that encompasses comprehensive home and community service(s) insurance verification, authorization and adherence to contract guidelines The Case Manager is responsible for the overall primary case management of those individuals who have been assigned to their caseload.
The Case Manager MUST demonstrate successful team participation, excellent written and verbal communication skills, advance computer/typing skills and Must have working knowledge of software applications, knowledge of local, state, and regional resources and willingness to establish community networks. The Case manager MUST be a mature person who is friendly, caring, can communicate well and has good organizational skills. The Case Manager MUST have a personal vehicle and maintain a valid Georgia driver?s license at all times.
The Case Manager MUST demonstrate a professional and personal commitment to advocacy, creativity, and ethics with an emphasis on promoting Independence. Promote and protects individual?s rights, especially their freedom of choice and the right to freely make and express preferences in choices regarding their daily living and care.
REQUIREMENTS:
Licensed as a registered nurse or have a BA or BS degree from an accredited college or university or bachelor?s degree case manager must have a degree in a health care or human services related discipline and must minimum of two years demonstrated and documented experience in healthcare service delivery or human services case management. Experience must be pertinent to the conditions and disabilities of the populations served by the ICWP and include work with adults, the disabled, traumatic brain injured or other special needs populations.
Job Types: Part-time, Contract
Pay: $22.46 - $24.68 per hour
